美國研究生留學(xué)簽證：ds表的填寫方法

1、passport number 護(hù)照號碼

2、place of issuance 護(hù)照的簽發(fā)地點(diǎn),city 城市,country 國家（國內(nèi)的,選擇china-mainland）,state/province 省份（直轄市可以不填）

3、issuring country 護(hù)照簽發(fā)國家（國內(nèi)的,選擇china-mainland）

4、issuance date (dd-month-yyyy) 護(hù)照簽發(fā)時間,這一項(xiàng)與普通ds156表格有一點(diǎn)區(qū)別,普通的156表格的日期格式為mmm,而網(wǎng)上的156,直接選擇就可以了。

5、expiration date (dd-month-yyyy) 護(hù)照過期時間（填寫方法同上）

6、surnames (as in passport) 護(hù)照上的姓（拼音）

7、first and middle names (as in passport)護(hù)照上的名（拼音）

8、other surnames used (maiden, religious, professional, aliases) 其他姓

9、other first and middle names used 其他名

10、date of birth (dd-month-yyyy) 出生日期（“月份”那一項(xiàng),直接選擇就可以了）

11、place of birth 出生地點(diǎn)

12、nationality 國籍（選擇china-mainland就可以了）

13、sex 性別（直接選）

14、national identification number 身份證號碼

15、home address (include apartment number, street, city, state or province, postal zone and country) 家庭地址（包括房間號碼,街道,城市,省份,郵編和國家）

16、電話號碼（包括家庭電話、辦公電話、移動電話、傳真號碼、工作傳真號碼、尋呼機(jī)pager number,有幾個填幾個,沒有就不填）

17、marital status 婚姻狀況（married——已婚, single (never married)——單身（從未結(jié)婚）, widowed——寡居, divorced——離婚, separated——分居）

18、 spouses full name (even if divorced or separated. include maiden name.) 配偶的全名（即使離婚或分居了也要填,包括婚前姓）——外國女人結(jié)婚后,可能把自己的姓氏改為丈夫的姓氏,所以這里才提出“婚前姓”,中國人不用考慮這一 點(diǎn)。

19、spouses dob (dd-month-yyyy) 配偶的出身日期（dob是date of birth的縮寫）

20、name and address of present employer or school 當(dāng)前學(xué)?；蛘吖椭鞯拿趾偷刂罚☉?yīng)屆生填寫學(xué)校地址即可。若是畢業(yè)一段時間了, 而且又沒有找到工作,可以不填）

21、present occupation (if retired, write "retired". if student, write "student".) 當(dāng)前的職業(yè)（如果退休了,填“retired", 如果是學(xué)生的話,填"student")

22、 when do you intend to arrive in the us? (provide specific date if known) 打算什么時候到美國？（如果知道具體日期的話,請注明具體日期） 填寫i20上的報到時間,好像也可以提前兩三天。

23、e-mail address 電子郵件地址

24、at what address will you be staying in the u.s.?在美國的居住地點(diǎn) （填寫i20上的學(xué)校地址即可）

25、 name and telephone numbers of person in u.s. who you will be staying with or visiting for tourism or business 邀請人的名字,你去拜訪的人是誰（對于學(xué)生來說,name填學(xué)校的名稱,電話填國際學(xué)生辦公室或者申請的院系的電話）

26、how long do you intend to stay in the u.s.? 你打算在美國停留多久？（i20上的學(xué)習(xí)時間）

27、what is the purpose of your trip? 此行的目的（f1就填study)

28、who will pay for your trip? 誰付錢？（對于學(xué)生來說,自費(fèi)的可以寫funds from family (或者是parents),獲得獎學(xué)金的可以寫funds from xxx university）

29、have you ever been in the u.s.? 曾經(jīng)去過美國嗎？

30、have you ever been issued a u.s. visa? 曾經(jīng)獲得過美國簽證嗎？

31、have you ever been refused a u.s. visa? 曾經(jīng)被拒簽過嗎？

32、do you intend to work in the u.s.? 有沒有在美國工作的意向？（學(xué)生肯定要填no）

33、do you intend to study in the u.s.? 有沒有在美國學(xué)習(xí)的意向？

34、names and relationships of persons traveling with you 隨行人員的名字,以及和你之間的關(guān)系（除攜行外,不要填寫。或者填寫none）

35、has your u.s. visa ever been cancelled or revoked? 您的美國簽證是否被吊銷過？（填no）

36、has anyone ever filed an immigrant visa petition on your behalf? 是否有人為您申請過美國移民簽證？（填no）

37、 are any of the following persons in the u.s. or do they have u.s. legal permanent residence or u.s. citizenship? mark yes or no and indicate that persons status in the u.s. (i.e., u.s. legal permanent resident, u.s. citizen, visiting, studying, working, etc.). 下列親屬中是否有人已在美國？他們是否是美國合法永久居民或美國公民？請注明“在美國”或“不在美國”。如選擇“在美國”,請注明其在美國的身份(即是美 國合法永久居民還是美國公民,是否正在美國訪問、留學(xué)、工作等)。

（1）、只在與自己有關(guān)的家庭成員前面選擇。無關(guān)的,比如無兄弟姐妹,就不用選擇

（2）、非直系親屬不用填,比如表姐妹、表兄弟。

39、was this application prepared by another person on your behalf? 申請表是否由他人代為填寫？（一般選no,不用填40項(xiàng)）

40、application prepared by 代寫人

41、簽名（對于網(wǎng)上填表來說,要先打印,然后手工簽名）

填表之后,點(diǎn)擊網(wǎng)頁下方的“continue",等一會兒,就會自動生成一個ds156表（共三頁）。檢查無誤、打印出來以后,在第一頁貼上照片,第二頁簽名就可以了。簽證的時候帶齊3頁
